Alastair is basically a demon extraordinaire who prefers spending his days down in hell breaking and converting the souls of the dead into taking pride and joy in breaking others as they were broken. Specially, it was Alastair's job to break the first seal which was, essentially, to make a "good man" torture the souls of others. This was Dean. His relationship to Sam is that through Dean, Alastair has had several run-ins with Sam, tortured his girlfriend at the time, Ruby, and was later killed by Sam as part of a push to get Sam to use his powers more fully (may have been unintentional on Alastair's part).

In short, he's a sadistic bastard and he loves it.
